Insurance Information

Below is a list of the insurances that we carry. Please have your insurance card with you at the time of your visit. Most insurance companies require us to collect co-pays. If your insurance company requires us to collect a co-pay and it is not paid at the time of the visit, there will be a charge of $10.00 applied to your account if it is not paid by the next day.

Please keep us informed of any changes in your address, telephone number and especially changes in your child’s insurance coverage. Please bring your insurance card with you. An insurance change requires us to scan the insurance card for placement in your chart. Our office policy is payment by cash, check, Visa, MasterCard, American Express or Discover at the time of the visit.

Please call the office to speak with the bookkeeper if there is a temporary problem that requires our cooperation.
